Claude Code fixes hook, MCP and secret-redaction gaps

Anthropic’s Claude Code 2.1.296 release fixes cases where disabled MCP servers could start in headless sessions, managed hooks could mishandle a denied tool call, and shared transcripts or debug logs could miss some secret values during redaction. It also changes several workflow and gateway controls. Teams using unattended or shared sessions should review the release and update process; the notes do not establish that a breach occurred.
